1. Original Passport (mandatory) + Photocopy
2. Original valid CNIC/NICOP/Smart Card (mandatory) + Photocopy

Note: In Case of Dual Nationality Original Foreign Passport along with photo copies (Mandatory)

For Children Under 18 Years:

1. Father’s Original valid Passport (mandatory) + Photocopy
2. Father’s Original valid CNIC/NICOP (mandatory) + Photocopy
3. Mother’s Original valid Passport (mandatory) + Photocopy
4. Mother’s Original valid CNIC/NICOP (mandatory) + Photocopy
5. Child Original Passport (mandatory) + Photocopy
6. Child Original valid Form B/NICOP/ Smart NICOP + Photocopy
7. Father and Mother Personal Presence along with child (Mandatory)
8. In case of separation custody papers from court duly attested From Ministry of Foreign Affairs Pakistan and UAE

For New Born Babies:

1. Child Original Birth Certificate Attested (mandatory) + Photocopy (Attested from Ministry of Foreign Affairs UAE)
2. Child Original NICOP (mandatory) + Photocopy
3. Father’s Original valid Passport (mandatory) + Photocopy
4. Father’s Original valid CNIC/NICOP (mandatory) + Photocopy
5. Mother’s Original valid Passport (mandatory) + Photocopy
6. Mother’s Original valid CNIC/NICOP (mandatory) + Photocopy
7. Father and Mother Personal Presence along with child (Mandatory)
8. If Father/Mother is other national need NOC from Consulate
9. Sworn Affidavit / Personal NOC Required if Father or Mother is foreign national attested from concerned consulate/Embassy
